Maui publishes an 'Affidavit on Application for Voter Registration Data' form but does not publish a flat fee online; requester must contact Elections Division for current quote. County rates-and-fees schedule (Appendix B) is published annually but does not break out voter file pricing line-item publicly.

Use restrictions
Election or government purposes only under HAR 3-177-160(a), and subsection (e) puts supporting or opposing a candidate first on the list of permitted purposes. A voter's full name, district and precinct designation and voter status are public for any purpose under subsection (b). Everything else may not be used for a commercial purpose, but subsection (f) provides that 'service bureaus may charge a fee for their services involving the use or disclosure of voter registration information that is not public under subsection (b) so long as the underlying election or government purpose is verified', and subsection (g)(4) repeats that proviso inside the sworn certification. Telephone number, email address and date of birth are excluded under subsection (c); residence address and mailing address are provided at a minimum. The application is a self-subscribing oath, and HRS 19-3.5 makes a knowingly false statement under a Title 17 oath a class C felony, which is a penalty for lying on the application, not for using the data.
