- Complete the Voter Registration Data Request form (CVR.pdf — https://sos.ks.gov/forms/Elections/CVR.pdf). Section A: requester name, address, email, phone. Section B (optional): filter by district and/or party affiliation — skip to receive all registered voters statewide. Sign the K.S.A. 25-2320a anti-commercial-use affirmation. Mail or email the completed form with check payment to the Elections Dept. in Topeka. Delivery typically by email (Excel/CSV) or physical CD. No online ordering portal.
